Ball Kid Scheduling Form

Hello Ball Kids!

Thank you for informing us as to your availability to work during the tournament.  We will be using this information to insure we have proper coverage for each day of the tournament.  Please fill out the below schedule, marking the days and times that you can work.

Please note the following:

• All ball kids must work a minimum of five sessions during the tournament.

• Informing us of your availability does not insure that you will be working a specific schedule, or that you will be working all sessions that you have volunteered for.

• Be sure to let us know of family or  plans for carpooling in the space provided.  We will work to have these schedules compatible.

• Your specific schedule will be available at uniform distribution.

• Adjustments to your schedule after the tournament has started will be very difficult.  Please check for any commitments, tests, or other conflicts before filling out this form.
